Accident Checklist

If you have an accident:

1. Check to see if anyone is injured.
2. Call 911 to report the accident and to request an ambulance if needed.
3. Get the name, address, phone number and the name of the insurance company from the other driver.
4. Get the name, address and phone number of any passengers or witnesses.
5. If you have a camera or video camera, take pictures of the scene.

After the accident:

1. Notify your own insurance company.
2. If you have been injured, see your doctor.
3. Take several pictures of any damage to your vehicle and property.
4. Take several pictures of any visible injuries to your body.
5. Call 225-888-3180 and speak to Michael Davis to schedule an appointment at your earliest convenience.
